Be Aware - In order to successfully submit this application you must have the following documents completed and digital files attached to the end of this form:

  1. FAFSA: All applicants must complete the Free Application for Federal Student Aid.
  2. Recommendations: Your recommendations can be sent to us with your application or they can be sent directly to us by the recommenders. It is your responsibility to make certain that the recommenders have followed through. Do not send more than two recommendations. (Please use the forms provided.)
  3. Tax Returns: Most recent TWO years of Federal Income Tax returns are required, front page only that shows the adjusted gross income. * BLACK OUT ALL SOCIAL SECURITY NUMBERS. (* Do not send Michigan tax forms.) * If parents are divorced, send the tax forms of the parent who claims you. * If your parents are on disability and do not fill out tax forms, send some type of government proof such as a letter that your parent has received in the past stating that he/she is on disability.
  4. College Financial Information: Financial Information Summary Sheet completed by the school you plan to attend. It will be returned to you to send in with your application.

If you have not completed the Financial Information Summary Sheet and received a copy back from the financial aid office, OR you have not received back your Letters of Recommendation, start the process by filling out the forms below.

Should you decide to use USPS to mail your application: Send in complete forms such as a recommendation cover sheet that states a letter will follow. Do not use staples on any of the application nor binders or folders of any type. If you do NOT reside in Jackson or Lenawee counties but go to a Jackson or Lenawee school, you do NOT qualify for a Sigmund Scholarship.

Send in your Application:
You can complete your application on-line OR send through the mail. If using the mail, the envelope MUST be postmarked on or before April 15 and mailed to: Sigmund Foundation, PO Box 1128, Jackson, MI 49204. (Without adequate postage, the application will be returned to you and you may miss the deadline. *There is no drop-off site.)
